Motor Vehicle Claim Forms

Requirement List when making a Motor Claim.

• A completed and signed Claim Form
• Copy of Drivers License (Owner and or the person driving at the time of incident
• Police Report
• Minimum of two Quotes from Authorised Garages  
• Photos if necessary
• Copy of any demands made by other parties for their vehicle/property
• Copy of towing fees (if applicable).
    
• Repairs to vehicle must not be carried forward without our approval.
